Step 3: Schedule Your Appointment

After your paperwork has been submitted, someone will let you know to contact our office to schedule your first appointment.

Important Insurance Information

If your insurance plan requires you to select a Primary Care Provider (PCP), please contact your insurance company at least one week before your appointment.

Your PCP must be listed as:

Failure to update your PCP before your appointment may result in your visit being delayed or rescheduled.

New Patient Visit $200

Follow-Up Visit $150

EKG $25 

Pap Smear $150

USCIS Immigration Consultation $300

We accept most major insurance plans, including Aetna, AmeriHealth, Braven Health, Cigna, Clover Health, Fidelis Care, GHI EmblemHealth, Horizon Blue Cross Blue Shield, Horizon NJ Health, Humana Medicare, Medicare, Meritain Health, Oxford, Oscar, QualCare, UnitedHealthcare Commercial, UMR, Wellcare Medicare, Wellpoint, and select AARP Medicare and Supplemental plans.

At this time, we do not participate with Aetna Better Health Medicaid, Aetna Family Care, Cigna LocalPlus IN Network, Medicaid without an HMO, Multiplan, Oxford Metro Network, UnitedHealthcare Community Plan (NJ Family Care), or UnitedHealthcare Dual Complete.
